Client Agreement
It is my choice to receive massage therapy. I am aware of the benefits and risks of massage and give my consent for massage. I understand that there is no implied or stated guarantee of success of effectiveness of individual techniques or series of appointments. I acknowledge that massage therapy is not a substitute for medical care, medical examination or diagnosis. I have stated all medical conditions that I am aware of and will inform my therapists of any changes in my health status.

Cancellation Policy:
We require a minimum of 24 hours notice to cancel an appointment. Failure to attend an appointment with less than 24 hours notice of cancellation or No Show will be charged 50% of the scheduled appointment fee which will be processed on the credit card retained on file to reserve appointment. 100% of that amount will go to the therapist scheduled to perform the services to cover their wait and travel time.

Late Arrivals:
It is important that our agreed appointment time and duration is adhered to. We appreciate that sometimes, things happen, and you may arrive a little later or need to leave a bit earlier than planned. This is fine, but we cannot make up the missed time by overrunning or adding time to subsequent appointments.

Confidential of medical records:
All client information and clinical records will be safeguarded by Dreamy Sun Massage, PLLC. and will remain confidential. Unless required to do so by law, we will not otherwise share, sell or distribute any of the information you provide to us without your consent.

Consultations and client treatment details will not be discussed with anyone other than the client, unless the client is under the age of 18 or has a care worker or guardian.

Boundaries:
This is an environment for professional, therapeutic massage.
*Requests for sexual activity will be viewed as solicitation and will be reported to the proper authorities. Any misconduct will not be tolerated.
